Adding a Document Checklist

Checklists are related questions that must be answered before a document can be fulfilled. The Checklist setup file provides a Question tab where questions for the checklist can be entered along with valid responses. The checklist is considered complete once all checklist items are signed off. Then the requirement fulfillment can be awarded.

Checklists can be set up for document sign-off and classroom training on documents for trainees and/or trainers:

Training Type Description

Sign-off Only

If a checklist is applied to a document which is used strictly for acknowledgement, or sign-off only, the trainee must complete the checklist prior to fulfilling the requirement. Once the trainee completes the checklist, the trainee will receive credit for the document. If a trainer (or manager) must also sign-off on the checklist, then the trainee will not receive credit for the document until AFTER the trainer has also completed the checklist.

Classroom Training

If a checklist is applied to a classroom training on the document for the trainee, the trainee must complete the checklist prior to fulfilling the requirement, but they cannot do so until after the training session has been marked as complete.

Once the trainee completes the checklist, the trainee will receive credit for the document. If a trainer (or manager) must also sign-off on the checklist, then the trainee will not receive credit for the document until AFTER the trainer has also completed the checklist.

Delinquent Checklists

If the document is past due, then the checklist will be added to the requirement in addition to any other checklists attached which may have questions asking why the trainee was late on their response (the trainee will be forced to justify why the document is past due upon checklist sign-off). This functionality is only applicable if the system administrator has set up Delinquent Checklist in your system.

Follow the steps below to add individual questions and pre-existing checklists to a document.

  1. In the document tree on the left side of the Document record, click the Certification Information link.
    Result: The document certification information is displayed.
  2. Click the No link next to Question Exists.
  3. Click Action > Add to add individual questions or select Add From Library to select a pre-defined checklist. For additional information on configuring pre-defined checklists, see Checklists.

Add

Complete the following fields when adding individual questions:

  • Question No. – Enter the question number.
  • Question – Enter the question.
  • Assign To – Click the drop down button and specify whether the question will be assigned to a trainer or to a trainee.
  • Question Response Set – Zoom to select the question response set for the question. For additional information on configuration, see Question Response Sets.
  • New Certification - Check this checkbox if this question will be for new certifications.
  • Revision Certification - Check this checkbox if this question will be for revision certifications.
  • Refresher Certification - Check this checkbox if this question will be for refresher certifications.

Add From Library

Complete the following fields when adding a pre-defined checklist:

  • Checklist Code – Zoom to select the pre-defined checklist.
  • Assign To – Click the drop down button and specify whether the checklist will be assigned to a trainer or to a trainee.
  • New Certification - Check this checkbox if this checklist will be for new certifications.
  • Revision Certification - Check this checkbox if this checklist will be for revision certifications.
  • Refresher Certification - Check this checkbox if this checklist will be for refresher certifications.
  1. Click the Save button.
